False Proceed Signals
This section of the Research Center features a compilation of Forms OMB No. 04-R-4028, U.S. Department of Transportation, Federal Railroad Administration, False Proceed Signal Report. All railroads in the U.S. that are subject to the regulations of the Federal Railroad Administration (FRA) must submit a False Proceed Signal Report to FRA within five (5) days after a false proceed occurs.
FRA defines a "false proceed" failure as "a failure of a system, device, or appliance to indicate or function as intended which results in less restriction than intended." For instance, if a signal is displaying a Green (PROCEED) signal into the next signal at Red (STOP), the Green signal is experiencing a "false proceed" failure.
IWT has submitted a number of Freedom of Information Act (FOIA) requests to the Federal Railroad Administration in Washington, D.C. for copies of all False Proceed Signal Reports submitted by U.S. railroads to FRA within specific time periods. Presently, this page contains all such reports sent by FRA to IWT in response to these requests for reports submitted by all U.S. railroads for the period from January 1, 1995 through May 3, 2004 (except 1999 records for BNSF, CSXT, NS, and UP). We have a number of FOIA requests pending, and will post them here as soon as they are received.
Each report has been transcribed into a computer database and sorted by a number of specific criteria. The contents of each report have been transcribed into the database verbatim, except for minor grammatical corrections. In addition, the names of each signal indication (i.e., CLEAR, APPROACH MEDIUM, etc.) have been capitalized for greater emphasis, and the names of each aspect color (i.e., Red, Yellow, Green, Red over Green, etc.) have been capitalized for clarity. No substantive changes have been made to any of the records appearing herein.
At the bottom of this page, photocopies of each report may be downloaded to compare with their database transcriptions, if desired.
As the following data is self-reported by the identified railroads, it cannot be guaranteed that all false proceed incidents were reported to FRA as required, or hence, that all appear here.
